Key Responsibilities:

The Licensed Practical Nurse (LPN) provides essential care to residents and patients in our health and rehabilitation center (skilled nursing facility), assisting them with daily living activities under the guidance of a registered nurse.


Responsibilities include, but are not limited to:
  • Assist in executing personalized treatment plans for each assigned resident.
  • Supervise Certified Nursing Assistants (CNAs) and contribute to their performance evaluations.
  • Manage the Falls Prevention Program and Pressure Ulcer Prevention Program.
  • Deliver nursing services as required.
  • Conduct and document comprehensive evaluations of each resident's medical condition upon admission and throughout their treatment journey.
  • Perform additional duties as assigned.

Qualifications:
  • Must hold a current, unencumbered, active state license to practice as an LPN.
  • Experience in skilled nursing or rehabilitation settings is preferred.
  • Must be qualified, compassionate, and committed to excellence in care.
