About the Role

We are seeking a highly skilled and experienced RN Clinical Manager to join our team at CenterWell Home Health. As a key member of our leadership team, you will be responsible for overseeing the clinical operations of our home health services, ensuring the highest quality of care for our patients.

Key Responsibilities
  • Develop and implement clinical policies and procedures to ensure compliance with regulatory requirements and Company standards.
  • Supervise and manage a team of clinical staff, including nurses, therapists, and home health aides, to ensure delivery of high-quality patient care.
  • Conduct regular audits and quality assurance activities to ensure compliance with regulatory requirements and Company standards.
  • Collaborate with the Branch Director and other stakeholders to develop and implement business strategies to drive growth and improve patient outcomes.
  • Provide education and training to clinical staff on new policies, procedures, and best practices.
  • Participate in performance improvement activities to identify areas for improvement and implement changes to enhance patient care and staff performance.
Requirements
  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR Card.
  • Valid drivers license, auto insurance and reliable transportation.
  • Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment.
  • Home health experience, required.
  • Management and people leadership experience, preferred (highly desired).
  • OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred.
  • Homecare Homebase (HCHB) experience, preferred.
  • CMS PDGM billing knowledge and/or experience, preferred.
