Key Responsibilities- Mentor and evaluate clinical training and competencies of advanced practice providers.
- Develop and disseminate policy and procedure guidelines, ensuring compliance with established protocols and documentation/billing requirements.
- Communicate clinical practice changes and updates to the team.
- Analyze data to identify trends and make recommendations for modifications of practice.
- Assist in the onboarding and ongoing education of advanced practice providers.
- Oversee and develop orientation programs for advanced practice providers in palliative and hospice care.
- Ensure ongoing updates to orientation programs as standards and best practice guidelines change.
- Conduct regular meetings with advanced practice providers to address clinical competency gaps and concerns.
- Complete routine visits to ensure compliance with clinical standards, protocols, and quality care.
- Partner with leadership to provide structured feedback on the progress, success, and challenges of advanced practice providers.
- Audit advanced practice provider documentation and competency in the EMR.
- Advise and ensure advanced practice provider involvement and communication with interdisciplinary groups to meet patient and family goals of care.
- Basic knowledge and understanding of hospice operations and performance improvement concepts.
- Create and monitor new advanced practice provider orientation programs.
- Serve on regional hospice and palliative care work groups as assigned.
- Provide community education on topics related to palliative and end-of-life care.
- Participate in ongoing professional development to maintain clinical competency.
- 5-10 years of relevant experience in hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, or medical-surgical nursing.
- Masters degree in nursing.
- Certification in hospice and palliative care nursing required within six months of hire if not already certified.
- Experience in training in palliative care and symptom management and/or recent hospital experience.
- Knowledge of program management is desired.
- Current, active, and unrestricted license as a Registered Nurse (RN) in NC and SC, and official recognition as an Advanced Practice Provider from the NC and SC Board of Nursing.
- Current and unrestricted DEA and Basic Life Support required.
