Job Title: Care Manager II

Join our team at Institute on Aging as a Care Manager II, where you will play a vital role in shaping the future of healthcare for older adults and individuals with disabilities.

About the Role:

We are seeking a highly skilled and compassionate Care Manager II to join our team. As a Care Manager II, you will be responsible for assessing the needs of clients with complex medical and psychosocial conditions, developing and implementing personalized care plans, and coordinating services to ensure the best possible outcomes.

Key Responsibilities:
  • Conduct comprehensive assessments of clients, including psychosocial, physical, and mental health needs, as well as environmental and spiritual factors.
  • Develop and implement client-centered care plans, taking into account program guidelines and policies.
  • Coordinate services, including home visits, hospital and skilled nursing facility visits, and escorts to medical appointments as needed.
  • Identify and arrange for community services, leveraging knowledge of Medicare, Medi-Cal, and other entitlement programs.
  • Establish and maintain relationships with clients, their support networks, and healthcare professionals, providing respect, dignity, and support.
  • Provide crisis intervention, advocacy, problem-solving, and therapeutic interventions as needed.
  • Document all case management activities, including progress notes, within 24-48 hours, and update care plans as necessary.
  • Maintain accurate and up-to-date records, adhering to a clear and consistent system of charting.
  • Evaluate client needs and follow up to ensure timely provision of services.
  • Educate clients and their support networks about available resources.
  • Establish and maintain open communication with community providers, including physicians and other healthcare professionals.
  • Monitor the quality and quantity of services provided by other involved providers.
  • Collaborate with the team to evaluate clients' ability to remain safely at home, coordinating placement as needed.
  • Participate in research studies and data collection, as required.
  • Contribute to ongoing efforts towards Continuous Quality Improvement.
  • Attend team and program meetings, activities, and problem-solving endeavors, contributing to open lines of communication.
Requirements:
  • M.S.W. (Masters in Social Work) degree required; LCSW preferred.
  • One year of experience working with disabled adults and/or older adults required.
  • Experience with and understanding of medical and psychosocial problems of functionally impaired adults and older adults.
  • Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders highly desired.
  • Exceptional communication and presentation skills, with the ability to relate to functionally impaired adults and older adults, their support systems, and healthcare professionals.
  • Case management skills and experience in the community healthcare delivery system.
  • Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
  • Computer literacy required; EPIC electronic medical records experience highly preferred.
Compensation:

Salary range: $64,789.25/annual. This amount is not necessarily reflective of actual compensation, which may vary based on experience, education, and other factors. Additional equity, benefits, and non-monetary compensation may be included.
