Overview:

This role involves a Clinical Pharmacist collaborating with healthcare professionals, including physicians and care managers, as part of a multidisciplinary medical team dedicated to enhancing patient outcomes while managing costs effectively.

The Clinical Pharmacist is responsible for providing therapeutic evaluations and targeted disease management for designated practices, with the goal of improving both patient and practice results.


Key Responsibilities:
1. Contribute clinical pharmacy expertise during the development of comprehensive care plans for patients.

2. Aid in selecting the most cost-efficient, evidence-based treatment plans for individual patients; monitor their progress and ensure appropriate follow-up.

3. Act as a drug information resource for healthcare providers and support personnel.

4. Oversee patient responses to therapies, identify potential drug interactions, and evaluate medications prescribed at dosages exceeding FDA recommendations for specific uses.

5. Provide specialized education and training in advanced pharmacotherapy, particularly focusing on chronic conditions.

6. Utilize available technology to enhance the retrieval of clinical data for monitoring and reviewing medication use.

7. Engage in initiatives aimed at improving outcomes for specific disease management programs, such as COPD, Asthma, CHF, and Diabetes.

8. Identify educational gaps in patient and family understanding of diagnoses and treatments, developing interventions with their input to address these gaps.9. Manage patients experiencing a decline in functional status.
10. Support pharmacy initiatives including Over the Counter (OTC) protocols, Preferred Drug Lists (PDL), and electronic prescribing efforts as outlined by the relevant program office.
11. Assist physicians in managing medication regimens for patients with chronic diseases, which may involve patient consultations, medication adjustments in collaboration with primary care providers, and conducting monitoring activities.
12. Conduct medication reconciliation assessments as requested by healthcare providers to optimize patients' medication regimens.
13. Serve as a resource for healthcare providers on general drug information and policy matters.

Physical Requirements:
This position is based in a clinical environment with potential exposure to infectious diseases and hazardous materials.
The work setting is climate-controlled, with occasional exposure to adverse weather conditions.
Flexibility in working hours is essential.
Requires intensive focus on tasks such as using computers and calculators.

Physical demands include:

Frequent movement within the healthcare facility, periods of standing, sitting, and walking; reaching and light lifting; manual dexterity; visual and auditory capabilities within normal limits.

Proficiency in electronic health records and various healthcare applications is necessary.

Maintain patient and family confidentiality in compliance with HIPAA regulations. Ability to operate a personal computer and familiarity with word processing, spreadsheet, and database software is required.

Ability to work independently, demonstrating sound judgment, discretion, and initiative to drive improvements.
Strong analytical and problem-solving skills, along with excellent interpersonal, written, and verbal communication abilities.
Outstanding organizational skills, professionalism, and diplomacy are essential.

Education & Experience Requirements:
Completion of an accredited College of Pharmacy program, clinical pharmacy residency, or equivalent experience and licensure.
A minimum of one year of recent experience as a clinical pharmacist, with involvement in formulary development, drug utilization review, and quality assurance activities.
Experience in Ambulatory Care is preferred.
Completion of a pharmacy practice or specialty residency (PGY1 or PGY2) is desirable.

Knowledge, Skills, and Abilities:

Comprehensive knowledge of pharmacy and its related disciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug utilization review, quality assurance, legal, regulatory, and practice standards.

Effective written and verbal communication skills; ability to collaborate within a healthcare team.
Proficiency in using database information systems and Microsoft Office Suite.
Capability to research and analyze medical literature, including drug information, disease states, and clinical practice guidelines.
