Overview:

This role involves a Clinical Pharmacist collaborating with healthcare professionals, including physicians and care managers, to enhance patient outcomes while managing costs effectively.

The Clinical Pharmacist is responsible for delivering therapeutic evaluations and managing specific diseases within designated practices, with the aim of improving both patient and practice results.


Key Responsibilities:
Contribute clinical pharmacy expertise during the development of interdisciplinary care strategies for patients.

Assist in identifying the most cost-efficient, evidence-based treatment plans for individual patients; oversee patients for necessary laboratory tests and follow-up recommendations.

Act as a drug information resource for healthcare providers and support personnel.

Monitor patient reactions to therapies, assess potential drug interactions, and evaluate medications prescribed at dosages exceeding FDA-approved indications.

Provide specialized education and training support to other healthcare professionals in advanced pharmacotherapy, particularly focusing on chronic disease management.

Utilize available technology to enhance the retrieval of clinical data for the assessment and review of medication utilization.

Engage in initiatives aimed at improving outcomes for targeted disease management programs, such as COPD, Asthma, CHF, and Diabetes. Identify knowledge gaps in patients and their families regarding diagnoses and treatments, and develop interventions with their input to address these gaps.

Manage patients experiencing a decline in functional status.
Support and coordinate pharmacy initiatives, including Over the Counter (OTC) Standing Orders, Preferred Drug Lists (PDL), and electronic prescribing efforts as outlined by relevant program offices.
Assist physicians in formulating and managing medication regimens for patients with chronic conditions (e.g., diabetes, asthma, CHF, etc.). This may involve direct patient interactions, adjusting medication dosages in collaboration with primary care providers, monitoring peak flow, and delivering other professional services.
Conduct medication reconciliation assessments as requested by physicians or case managers to optimize patient medication regimens.
Serve as a resource for physicians and case managers regarding general drug information and policy matters.

Physical Requirements:
Work in a physician practice setting with potential exposure to infectious diseases and hazardous materials.
The environment is climate-controlled, with occasional exposure to adverse weather conditions.
Flexibility in working hours is required.
Engage in intensive tasks requiring machine operation, such as using computers and calculators.

Physical demands include:

frequent movement throughout the healthcare facility, with periods of standing, sitting, and walking; reaching and light lifting; finger dexterity; visual and auditory capabilities within normal limits.

Proficiency in electronic health records and various healthcare applications is essential.

Maintain patient and family confidentiality in accordance with HIPAA regulations. Ability to operate a personal computer and familiarity with word processing, spreadsheet, and database software is required.

Ability to work independently, exercising sound judgment, discretion, and initiative to drive improvements.
Demonstrate excellent analytical and problem-solving skills, along with strong interpersonal, written, and verbal communication abilities.
Exhibit exceptional organizational skills, professionalism, and diplomacy.

Education & Experience Requirements:
Graduation from an accredited College of Pharmacy, completion of a clinical pharmacy residency, or equivalent experience and licensure.
A minimum of one year of recent clinical pharmacy experience, including involvement in formulary development, drug utilization evaluation, and quality assurance activities.
Ambulatory care experience is preferred.
Completion of a pharmacy practice or specialty residency (PGY1 or PGY2) is desirable.

Knowledge, Skills, and Abilities:

Comprehensive knowledge of pharmacy and its related fields (therapeutics, pharmacology, physical pharmacy), formulary development, drug utilization review, quality assurance, legal, regulatory, and standards of practice.

Strong written and verbal communication skills; ability to integrate into a healthcare team.
Proficiency in utilizing database information systems and Microsoft Office Suite.
Ability to research and analyze medical literature, including drug information, disease states, and clinical practice guidelines.
