Overview:

This role involves a Clinical Pharmacist who collaborates with healthcare professionals, including physicians and care managers, as part of a dedicated medical team aimed at enhancing patient health outcomes while managing costs effectively.

The Clinical Pharmacist delivers therapeutic evaluations and disease management strategies for designated practices, striving to elevate both patient and practice results.


Key Responsibilities:
Contribute clinical pharmacy expertise in the development of interdisciplinary care strategies for patients.

Assist in identifying the most cost-efficient, evidence-based treatment plans for individual patients; monitor patient progress and ensure appropriate follow-up lab work and recommendations.

Act as a reliable drug information resource for healthcare providers and support personnel.

Evaluate patient responses to treatments, assess potential drug interactions, and review medications prescribed at dosages exceeding FDA-approved levels for specific conditions.

Provide specialized education and training in advanced pharmacotherapy, particularly focusing on chronic disease management.

Utilize available technology to enhance the retrieval of clinical data for monitoring and reviewing medication usage.

Engage in initiatives aimed at improving health outcomes for targeted disease management programs, such as COPD, Asthma, CHF, and Diabetes. Identify educational gaps in patient and family understanding of diagnoses and treatments, and develop interventions with their input to address these gaps.

Manage patients experiencing recent declines in functional abilities.
Support and coordinate pharmacy initiatives, including Over the Counter (OTC) protocols, Preferred Drug Lists (PDL), and electronic prescribing efforts as directed by relevant program offices.
Assist healthcare providers in managing medication regimens for patients with chronic conditions (e.g., diabetes, asthma, CHF). This may involve direct patient interactions, medication dosage adjustments in collaboration with primary care providers, and conducting peak flow assessments.
Conduct medication reconciliation assessments as requested by healthcare providers and case managers to optimize patient medication regimens.
Serve as a valuable resource for healthcare providers and case managers regarding general drug information and policy matters.

Physical Requirements:
Work in a clinical environment with potential exposure to infectious diseases and hazardous materials.
Maintain flexibility in work hours.
Engage in tasks requiring intensive focus, such as using computers and calculators.

Physical demands include:

frequent mobility within the healthcare setting, alternating between standing, sitting, and walking; reaching and light lifting; dexterity in fingers; and normal visual and auditory capabilities.

Proficiency in electronic health records and various healthcare applications is essential.

Maintain patient and family confidentiality in compliance with HIPAA regulations. Ability to operate personal computers and familiarity with word processing, spreadsheet, and database software.

Work independently while exercising sound judgment, discretion, and initiative to drive improvements.
Demonstrate strong analytical and problem-solving skills, along with excellent interpersonal, written, and verbal communication abilities.
Exhibit outstanding organizational skills, professionalism, and diplomacy.

Qualifications:
Education and formal training from an accredited College of Pharmacy, with completion of a clinical pharmacy residency or equivalent experience and licensure.
A minimum of one year of recent experience as a clinical pharmacist, including involvement in formulary development, drug utilization review, and quality assurance initiatives.
Ambulatory care experience is preferred.
Completion of a pharmacy practice or specialty residency (PGY1 or PGY2) is advantageous.

Knowledge, Skills, and Abilities:

Comprehensive understanding of pharmacy and its associated disciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug utilization review, quality assurance, legal, regulatory, and practice standards.

Strong written and verbal communication skills; ability to collaborate effectively within a healthcare team.
Proficiency in utilizing database management systems and Microsoft Office Suite.
Capability to research and analyze medical literature, including drug information, disease states, and clinical practice guidelines.
