Clinical Care Coordinator

About the Role:
The Clinical Care Coordinator plays a vital part in our team at CenterWell Home Health. As a Medical Matron, you will be responsible for overseeing the development and implementation of patient care plans, ensuring seamless coordination between healthcare professionals, and promoting optimal clinical outcomes. Your expertise will drive our commitment to whole-person health, addressing the physical, emotional, and social well-being of our patients.
Key Responsibilities:
- Develop and oversee patient care plans, ensuring alignment with physician-approved treatment plans.
- Collaborate with interdisciplinary teams to coordinate patient care, including nurses, therapists, and home health aides.
- Routinely complete start-of-care, recertification, and discharge OASIS visits, adapting to individual patient needs as required.
- Maintain focus on healthcare quality and affordability initiatives, adhering to established productivity standards and performance metrics.
Requirements:
To excel in this role, you should possess:
- A Bachelor of Science degree in Nursing (BSN) or equivalent experience.
- At least 2 years of clinical home care experience, preferably with a minimum of one year in a clinical management role.
- Knowledge of state and federal home health agency regulations, as well as compliance standards and procedures.
- Demonstrated proficiency in implementing clinical policies and procedures, particularly those related to PDGM.
- Excellent communication and interpersonal skills, with the ability to read, write, and speak fluent English.
