The MDS Coordinator plays a vital role in the healthcare delivery system, ensuring compliance with regulatory standards while optimizing reimbursement for patient care services. This position is essential in facilitating the Minimum Data Set (MDS) assessments and Care Area Assessments (CAAs).
Key Responsibilities:- Patient Care Planning: Collaborate in developing tailored care plans that address individual patient needs and anticipated outcomes in accordance with Federal and State regulations.
- Interdisciplinary Collaboration: Engage in the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, adhering to RAI guidelines for MDS completion, computer encoding, and electronic submission.
- Patient Condition Monitoring: Identify and report changes in patient conditions, coordinating with the interdisciplinary team to schedule and conduct necessary assessments.
- Staff Training: Provide training on Activities of Daily Living for new hires and facilitate ongoing education for existing staff members.
- Continuous Education: Attend educational sessions to stay updated on current RAI and Prospective Payment System regulations.

Qualifications:- Must possess a current Registered Nurse license in good standing.

- Knowledge of general, rehabilitative, and restorative nursing practices, as well as the laws and regulations governing long-term care, is necessary.
- Position Type: Full Time
- Pay Range: $40-$45 per hour
