Key Responsibilities:- Patient Care Planning: Collaborate in developing tailored care plans that address individual patient needs and anticipated outcomes in accordance with Federal and State regulations.
- Interdisciplinary Collaboration: Engage in the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, adhering to RAI guidelines for MDS completion, computer encoding, and electronic submission.
- Patient Condition Monitoring: Identify and report changes in patient conditions, coordinating with the interdisciplinary team to schedule and conduct necessary assessments.
- Staff Training: Provide training on Activities of Daily Living for new hires and facilitate ongoing education for existing staff members.
- Continuous Education: Attend educational sessions to stay updated on current RAI and Prospective Payment System regulations.

Qualifications:- Must possess a current Registered Nurse license in good standing.
- Previous experience in a hospital, long-term care facility, or similar healthcare environment is required.
- Experience in completing MDS assessments and Care Plans is essential.
- Knowledge of general, rehabilitative, and restorative nursing practices, as well as the laws and regulations governing long-term care, is necessary.
- Position Type: Full Time
- Pay Range: $40-$45 per hour
