Job Title: Director of MDS

We are seeking a highly skilled and compassionate Director of MDS to join our team at Royal Gardens Healthcare. As a key member of our healthcare services team, you will be responsible for conducting and coordinating the development, completion, and transmission of resident assessments in accordance with state and facility requirements.

Key Responsibilities:

  • Conduct and coordinate the development and completion of preliminary and comprehensive resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Ensure that care plans include measurable objectives and timetables to meet residents' medical, nursing, mental, and psychosocial needs.
  • Collaborate with healthcare professionals to assess residents' conditions and develop comprehensive care plans.
  • Ensure timely completion of initial resident assessments and updates to care plans as needed.
  • Supervise and coordinate the interviewing of residents for assessments.
  • Evaluate residents' conditions and pertinent medical data to determine any need to amend admission assessments.

Requirements:

  • 2-4 years' experience providing direct care to multiple patients/residents, preferably in a Skilled Nursing environment.
  • Nursing degree from an accredited college or graduation from an approved LVN or RN program.
  • Current, unencumbered license to practice as an LVN in California; RN license preferred.
  • Current valid CPR certification.
