Job Title: Home Health Nurse

We are seeking a skilled and compassionate Home Health Nurse to join our team at WVU Medicine. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' physical and psychosocial needs, identifying areas for improvement and developing strategies for care.
  • Administer medications and treatments as ordered by physicians, ensuring accurate and timely delivery.
  • Monitor patients' progress, reporting any changes or concerns to the healthcare team.
  • Provide education and support to patients and families on disease management, self-care, and health promotion.
  • Document patient care and progress in accordance with regulatory requirements and hospital policies.
Requirements:
  • Current licensure as a Registered Nurse in the state of West Virginia or ability to obtain licensure within 6 months.
  • 1+ year of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with flexibility to adapt to changing priorities and situations.
  • Basic Life Support (BLS) certification.
