Job Title: Home Health Nurse

We are seeking a skilled and compassionate Home Health Nurse to join our team at WVU Medicine. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that meet patients' unique needs and goals.
  • Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
  • Current licensure as a Registered Nurse in the state of West Virginia or ability to obtain licensure within 6 months.
  • Minimum 1 year of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
  • Basic computer skills and ability to navigate electronic medical records.
