We are seeking a skilled and compassionate Registered Nurse to join our team at Preston Memorial Home Health. As a Registered Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Responsibilities- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Provide direct and indirect supervision to home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
- Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits per policy and payer requirements.
- Communicate effectively with patients, families, and healthcare team members to ensure quality care and service excellence.
- Participate in on-call rotation and weekend rotation as needed to meet patient needs.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
