Job Summary

We are seeking a skilled and compassionate Home Health Nurse to join our team at WVU Medicine. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with patients, families, and healthcare teams to develop and implement personalized care plans.

Key Responsibilities
  • Provide direct and indirect nursing care to patients in their homes, including assessing, planning, implementing, and evaluating patient care.
  • Collaborate with patients, families, and healthcare teams to develop and implement care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and psychosocial needs, identifying areas for improvement and developing strategies to address them.
  • Administer medications and treatments as ordered by physicians, ensuring patient safety and comfort.
  • Document patient care and progress in a timely and accurate manner, using electronic medical records and other tools as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of West Virginia or a compact state.
  • Valid driver's license and reliable transportation.
  • CPR certification within 30 days of hire date.
  • One year of experience in home care nursing and two years of prior nursing experience preferred.
