Job Summary

Lifeline Connections is seeking a skilled Mental Health Case Manager to join our team. As a Mental Health Case Manager, you will provide community-based mental health services to patients, working under the supervision of the Mental Health Services Program Director and Program Supervisor.

Key Responsibilities
  1. Provide community-based case management services to patients assigned to you, in accordance with the individual service plan and evaluation recommendations.
  2. Work with patients to create and meet individual service plan goals and activities that are strength-based and meet their unique needs.
  3. Collaborate with the treatment team to provide comprehensive care to patients.
  4. Network with community agencies and service providers to identify and connect patients with available resources.
  5. Represent Lifeline Connections in the community at various meetings and clinical reviews, with approval from the supervisor.
Requirements
  • Bachelor's degree in mental health counseling or a related social service science.
  • Washington State Department of Health credentials.
  • At least one year of paid or voluntary experience working with adults or youth living with co-occurring mental health and substance use, or at least one year of experience working with adults in a mental health treatment facility.
  • Familiarity with Washington State regulations related to the provision of mental health services (RCW 71.05).
Working Environment

This position requires the ability to travel within the community by private vehicle to meet patients in their own environment. The position also requires the ability to assist patients with daily life skills activities, such as cooking, laundry, and cleaning.

Supervision

Immediate supervision will be provided by the Program Director and Program Supervisor.
