We are seeking a skilled RN Case Manager to join our team at Ascension Living. As a RN Case Manager, you will be responsible for providing direct nursing care to patients in a long-term care setting, implementing and monitoring patient care plans, and serving as a primary coordinator of all disciplines for well-coordinated patient care.
Responsibilities- Provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans, ensuring that patients receive high-quality, patient-centered care.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care, working closely with other healthcare professionals to ensure seamless care transitions.
- Note and carry out physician and nursing orders, ensuring that patients receive timely and effective care.
- Assure resident safety by identifying safety hazards and emergency situations and initiating corrective action as appropriate.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team, ensuring a smooth transition to post-acute care.
- Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date required.
- Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
