The Registered Nurse - Case Manager is responsible for planning, organizing, and directing home care services. This role requires a strong emphasis on community health education and experience. The ideal candidate will have a graduate degree in nursing and possess licensure in the state.
Key Responsibilities:
Conduct initial assessments of patients and families to determine home care needs.
Develop and implement plans of care that establish goals based on nursing diagnosis and incorporate therapeutic, preventative, and rehabilitative nursing actions.
Administer medications and treatments as prescribed by physicians.
Counsel patients and families in meeting nursing and related needs.
Provide health care instructions to patients as appropriate per assessment and plan of care.
Requirements:
Graduate degree in nursing.
Licensure in the state.
CPR certification and valid driver's license with automobile insurance.
Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
Ability to tolerate prolonged or considerable walking or standing and lift 25 pounds.
Available during normal working hours 8:00am-5:00pm Monday through Friday and understand on-call availability.
