RN Field Case Manager

As a Field Case Manager with Mindlance, you will play a critical role in facilitating progressive medical treatment and rehabilitation for injured workers. This position requires strong communication and problem-solving skills, as well as the ability to work independently and manage multiple priorities simultaneously.
Key Responsibilities
Assess and analyze injured workers' medical and vocational status
Develop progressive plans of care to facilitate medical recovery and return to work
Manage field case management referrals, including meetings with attending physicians, injured workers, and employers
Clarify medical and vocational claim-related information through in-person and telephonic communication
Collaborate with healthcare teams to provide ongoing case management services
Monitor injured workers' progress and coordinate necessary referrals and services
Requirements
Current state RN licensure required
Bachelor's degree in a health or human services field preferred
Certified Case Manager (CCM) or Certified Disability Manager Specialist (CDMS) certification required
Three years of home health, occupational health, or nursing case management experience preferred
Work Environment
This position requires 70% travel and involves working with various stakeholders, including patients, caregivers, attorneys, employers, and healthcare providers. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work in a fast-paced environment.
