Role Summary:
The IDD Care Management Team Leader is responsible for overseeing and guiding the operations of Care Management services, ensuring the delivery of high-quality care and support to individuals with mental health, substance use, and developmental disabilities. This role involves leading the administrative, clinical, financial, and employee relations aspects while ensuring adherence to company, state, and federal regulations. The Team Leader will directly supervise care managers and care manager extenders.
Compensation: $65,000-$75,000

Sign-on Incentive: $5,000

Qualifications:
  • A valid license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, such as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA), or a Registered Nurse (RN) license from the North Carolina Board of Nursing.
Experience Requirements:
  • A minimum of three years of experience in care management, case management, or care coordination for the target population.
Key Responsibilities:
  • Leadership in Care Management: Direct and supervise a regional team of care managers and extenders, fostering collaboration across the organization to develop and implement effective workflows, policies, and documentation standards that enhance comprehensive care.
  • Staff Development: Ensure optimal resource utilization to achieve regional and organizational objectives, oversee daily operations, manage recruitment, and facilitate staff training and development.
  • Commitment to Best Practices: Drive continuous improvement and innovation in care management interventions, staying informed about current best practices and sharing knowledge with the team.
  • Cross-Department Collaboration: Work collaboratively with various departments within RHA to ensure successful implementation of Care Management services.
  • Person-Centered Care: Uphold a philosophy focused on individual strengths, needs, and preferences, promoting health, wellness, and safety through active participation of individuals and their support systems.
  • Quality Assurance: Utilize data strategically to enhance the delivery of Care Management services, ensuring compliance with performance standards and regulatory requirements.
  • Safety and Compliance: Implement health and safety policies and maintain an ethical culture in alignment with RHA's Code of Conduct.
  • Financial Oversight: Collaborate with the Executive Director to manage and analyze program budgets, ensuring operations align with financial targets.
  • Effective Communication: Maintain open lines of communication with staff, colleagues, and external partners, fostering a culture of feedback and collaboration.
  • Ethics and Compliance: Uphold ethical standards and participate in investigations related to compliance issues, ensuring protection for individuals served.
Supervisory Duties:
  • Direct supervision of up to eight care managers and two extenders per manager, with responsibilities including hiring, training, performance reviews, and conflict resolution.
