IDD Care Management Team Leader

Position Overview:
The IDD Care Management Team Leader is responsible for overseeing the operations of Care Management services within RHA Health Services. This role is crucial for ensuring high-quality care and support for individuals with intellectual and developmental disabilities, as well as those facing mental health and substance use challenges.
Key Responsibilities:
- Leadership: Directly supervise a regional team comprising care managers and care manager extenders, fostering a collaborative and productive work environment.
- Service Delivery: Ensure the effective implementation of care management services, adhering to company, state, and federal policies.
- Staff Development: Oversee recruitment, training, and performance evaluations to maintain a skilled and motivated team.
- Quality Assurance: Utilize data and metrics to monitor service delivery and ensure compliance with established standards.
- Cross-Functional Collaboration: Work closely with various departments to enhance care management processes and outcomes.
- Financial Oversight: Collaborate with the Executive Director to manage program budgets and ensure operational efficiency.
- Ethics and Compliance: Uphold the organization's ethical standards and ensure adherence to compliance protocols.
Qualifications:
- A valid license or certification in a relevant human service profession.
- A minimum of three years of experience in care management or related fields.
Compensation:
$65,000-$75,000 with a sign-on bonus of $5,000.
