Healthcare Liaison Specialist

Relationship Management:
Develop and maintain strong relationships with healthcare providers, community organizations, and patients to promote home health and hospice services.
Act as a liaison between patients, families, and healthcare providers to address concerns, provide information, and facilitate care transitions.
Value-Based Care Support:
Assist in the implementation and management of value-based care models focusing on improving patient outcomes and reducing costs.
Monitor and analyze performance metrics related to quality, effectiveness of value-based care initiatives and home health and hospice services.
Care Coordination:
Collaborate with interdisciplinary teams to ensure the delivery of high-quality, patient-centered care.
Facilitate communication between various healthcare providers, LTC facilities, and healthcare utilization.
Community Outreach:
Represent Eventus WholeHealth in professional meetings to raise awareness about home health, hospice services, and value-based care.
Build and nurture relationships with referral sources.
Quality Assurance:
Monitor and address any issues or concerns promptly to ensure a positive experience.
Track and report on key performance indicators related to referrals, patient outcomes, and service utilization.
Administrative Duties:
Maintain accurate and up-to-date records of interactions, referrals, and patient information.
Assist with the preparation of reports and documentation as needed.
Qualifications:
Education:
degree or certification in Healthcare, LPN, ADN, certified medical assistant, or related field.
Experience:
Minimum of 2-3 years of experience in home health, hospice, or a related healthcare field. Previous experience in a liaison or care coordination in long-term care facilities and reimbursement is highly desirable.
Skills:
Excellent communication and interpersonal skills; strong organizational abilities; proficiency in Microsoft Office and electronic health records (EHR) systems.
Knowledge:
Understanding of home health and hospice care, including regulatory requirements and best practices. Familiarity with community resources, support services, and Value-Based Care
