Position Overview

The Cow Creek Band of Umpqua Tribe of Indians is in search of a committed and skilled individual to fulfill the role of APD Tribal Navigator. This pivotal position requires collaboration with the Aging and People with Disabilities (APD) and the Tribal Elders Coordinator to provide vital services and assistance to our tribal community members. The role is multifaceted, focusing on delivering direct support and navigating the intricate landscape of care and services for our elders. This position is not remote and necessitates a proactive approach to address the distinct needs of our community.

Key Responsibilities
  • Serve as a liaison and coordinate efforts with APD/AAA.
  • Collaborate with the Tribal Elders Coordinator to evaluate the needs of elders.
  • Assist in the formulation and oversight of service plans and providers.
  • Recruit and oversee in-home caregivers and clients.
  • Evaluate and mitigate risks while developing contingency plans.
  • Advocate for the needs of the community with other Tribal programs.
  • Identify additional resources to meet community needs.
  • Conduct pre-screening and ongoing assessments of needs.
  • Engage in assessments and home visits, both independently and alongside the APD case manager.
  • Participate in weekly and monthly care coordination meetings with APD and service providers.
  • Maintain organized case notes and documentation.
  • Provide quarterly reports to DHS APD.
  • Attend necessary training sessions as required.
  • Develop and update policies and procedures for the program.
  • Possess knowledge of Medicaid and long-term care facility requirements.
Qualifications
  • Bachelor's Degree in Behavioral Science, Social Science, or a closely related field; OR
  • Associate's degree in a related field with one year of experience in human services; OR
  • Five years of experience in human services casework and service provision.
  • Demonstrated effective interpersonal communication skills.
  • Proficient in office procedures, office equipment, Microix, and word processing/computer skills, along with knowledge of government program regulations.
  • Experience in assessing elder care needs and formulating appropriate care plans.
  • Familiarity with long-term care programs, including in-home services, community-based care facilities, and nursing homes.
  • Knowledge of local resources and experience in coordinating services with other individuals and agencies.
  • Experience in preparing and maintaining detailed documentation of activities related to individual cases.
  • Ability to work effectively in a team environment.
Working Conditions

This role frequently interacts with individuals who may be hostile, depressed, sensory impaired, cognitively or developmentally impaired, combative, terminally ill, or emotionally distressed. Occasional travel in adverse weather conditions is required, along with frequent travel to remote, rural areas. The position may involve encounters with animals at residences. A valid driver's license and a clean driving record are essential, as is the ability to work a standard 40-hour week.
