Job SummaryWe are seeking a Medical Social Worker to join our team, responsible for assessing and evaluating patient and family needs, developing and implementing social service plans of care, and coordinating ancillary client care services.
Responsibilities- Assess and evaluate patient and family needs to provide quality care to assigned patients
- Develop, discuss, and communicate a realistic plan of care for each client, including caregiver involvement
- Implement social service plan of care and procedures as necessary to ensure safe and effective quality of care for clients
- Evaluate the effectiveness of implemented plan of care, including identified problems, care provided, and client responses
- Maintain up-to-date and accurate documentation of social services provided
- Coordinate ancillary client care services with physicians, providers, and team members to ensure safe and effective client and family care
- Master's Degree in Social Work required
- Two years of experience in human services and/or social work in a medical environment required
- Prior experience in home care preferred
- Valid Driver's License
