About the Role

Falcons Healthcare is seeking a compassionate and skilled Certified Nurse Aide to join our Home Health team. As a Certified Nurse Aide, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.

Key Responsibilities
  • Provide personal care and incidental activities of daily living for patients of all ages, including toileting and elimination, bathing, grooming, and oral hygiene.
  • Utilize infection control measures such as hand washing and use of personal protective equipment.
  • Observe and report the condition of the patient/client to the appropriate supervisor.
  • Work in a team environment to effectively deliver care and assistance to patients and staff.
  • Assist with ambulation, transfers, and range of motion exercises.
  • Perform patient-related procedures such as measuring and documenting intake and output, blood glucose monitoring, and vital sign measurements.
Requirements
  • Must be 18 years of age.
  • High School diploma or equivalent and successful completion of Nursing Assistant course and Nurse Assistant certification as required by the state in which employed.
  • A student RN in the last year of school or an unlicensed RN/LPN/LVN graduate may be eligible for employment as a Nurses Aide as allowable under state law/regulation.
  • 1 year experience in a comparable job classification and/or as required by the contracted facility.
  • Home Care or Hospice experience preferred.
  • Proof of Covid vaccine, medical exemption or religious exemption.
