- Develop and implement personalized physical therapy plans for patients with diverse needs, taking into account their unique goals and objectives.
- Evaluate patients' physical abilities and create targeted training objectives to help them achieve optimal health outcomes.
- Collaborate with physicians and healthcare professionals to design effective treatment programs that meet the needs of our patients.
- Supervise and guide Physical Therapy Assistants and Home Health Aide staff to ensure high-quality care and consistent patient outcomes.
- Accurately document patient progress and observations, providing valuable insights for future treatment plans.
- Accredited Physical Therapy degree from an accredited institution.
- At least one year of experience in physical therapy, with a strong understanding of home health care principles.
- Current and unrestricted Physical Therapy license, as well as current CPR certification.
- Strong organizational and communication skills, with the ability to work effectively in a team environment.
