Overview

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a national leader in home-based care, Enhabit is committed to expanding what's possible for patient care in the home.

As a Case Manager, you will be the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode. You will consult with the physician and office staff as needed to ensure accurate and timely patient care.

Responsibilities
  • Perform visits and coordinate client care
  • Oversee the frequency of visits for the episode
  • Consult with the physician and office staff as needed
  • Perform accurate OASIS collection and ensure medication profiles are current
  • Attend weekly case conference and monthly case manager meetings
Qualifications
  1. Current licensure as a Registered Nurse in the state of employment
  2. Minimum of one year of clinical experience preferred
  3. Demonstrate knowledge and skill in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
