Position Overview

Under the supervision of a Licensed Professional (e.g., Physician or Nurse Practitioner), the Licensed Practical Nurse (LPN) plays a vital role in delivering patient care by employing the nursing process. This includes data collection and assessment, prioritizing patient needs, and evaluating the effectiveness of nursing interventions.

The LPN is responsible for coordinating and directing patient education activities and ensuring seamless care delivery in collaboration with the healthcare team. Compliance with documentation standards is essential within the scope of the Medical Assistant and LPN roles.

Key Responsibilities
  • Assess and analyze patient data, evaluate conditions, and collaborate with providers to develop and maintain comprehensive care plans.
  • Provide education to patients, families, and staff, in partnership with other healthcare professionals, under the guidance of the provider or manager.
  • Coordinate patient care with various healthcare entities in alignment with the established care plan.
  • Administer medications safely, adhering to organizational policies and state regulations.
  • LPNs in leadership roles act as subject matter experts and serve as clinical mentors within the practice.

Licensing/Certification Requirements
Current Licensed Practical Nurse (LPN) from the state of employment or LPN applicant (required)
BLS certification is preferred upon hire and required within 30 days and/or prior to independent patient care
