Key Responsibilities
  • Schedule and ensure timely completion of patient assessment visits, including all Oasis visits.
  • Assign and schedule clinicians efficiently, optimizing geography and matching skills with required care.
  • Collaborate with field clinicians to guarantee all patient visits are arranged and executed as directed.
  • Manage inquiries from patients and field staff regarding scheduling matters.
  • Refer clinical and performance-related concerns to the clinical manager.
  • Monitor hospitalized patients and ensure follow-up communication as necessary.
  • Support the clinical manager and facilitate timely communication with patients, clinicians, referral sources, and other office personnel.
  • Generate relevant reports and respond to workflow, taking appropriate actions.
  • Assist with the transfer of patients between care centers and/or hospice services.
  • Maintain the supply closet, ensuring supplies are current and appropriately packaged, and act as a liaison for patient supply needs.
  • Perform additional duties as assigned.
Qualifications
  • High School diploma or GED equivalent.
Preferred Qualifications
  • One year of scheduling experience in a healthcare setting.
  • One year of administrative experience in a healthcare environment.
