Nurse Care Coordinator

We are seeking a compassionate and skilled Registered Nurse to join our team at Elite Home Health. As a Registered Nurse in Home Health, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Conduct comprehensive assessments of patients in their homes to determine their nursing needs and develop individualized care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices in home health care.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
