Job Title: Licensed Practical Nurse - Home Health

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Nurse On Call, a part of LHC Group. As a home health nurse, you will provide quality patient care and perform technical skilled care in the patient's home.

Job Summary:

The LPN in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home. This position requires a strong commitment to delivering exceptional patient care and working collaboratively with the healthcare team.

Responsibilities:
  • Provide quality patient care and perform technical skilled care in the patient's home.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Monitor and report patient progress to the supervising nurse and/or physician.
  • Provide education and support to patients and their families on disease management and self-care.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
