As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents. Your primary responsibility will be to promote the health and wellness of our residents through various means, including conducting regular wellness visits, assessing and monitoring residents' health, and providing education and support to residents and their families.
Responsibilities
- Conduct regular wellness visits for all residents to assess their health and identify any potential issues.
- Monitor and document residents' health status, including vital signs and medication administration.
- Develop and implement individualized care plans to promote residents' health and wellness.
- Collaborate with the Resident Care Director and other healthcare professionals to ensure seamless care delivery.
- Provide education and support to residents and their families on health-related topics, including disease management and prevention.
- Participate in quality improvement initiatives to enhance the overall quality of care.
Requirements
- Graduate of an accredited nursing program.
- Current state license as a Registered Nurse (RN).
- Minimum of one year of experience in a healthcare setting, preferably in senior care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
