Job Title: Per Diem LPN

Job Summary:
EverHome Healthcare is seeking a skilled and compassionate Per Diem LPN to join our team. As a Per Diem LPN, you will provide high-quality nursing care to clients in their homes, promoting independence and well-being.

Key Responsibilities:
Provide home health visits to clients, assessing their needs and developing individualized care plans.
Admit clients to home health services, completing start of care documents and preparing and submitting clinical notes.
Collaborate with the Registered Nurse Case Manager to ensure seamless care delivery.

Requirements:
Licensed Practical Nurse (LPN) in the State of Washington.
3 years of clinical experience, preferably in rehabilitation, home care, or hospice.
Ability to practice independently and demonstrate problem-solving skills.
Excellent written and verbal communication skills.
Reliable vehicle for use when traveling to work sites.
Valid Washington State Driver's License and proof of current auto liability insurance.
