About This Opportunity

Sunrise Senior Living is seeking a dedicated and compassionate Wellness Nurse to join our team. In this role, you will be responsible for promoting the overall well-being of our residents, providing personalized care and support to help them thrive.

Responsibilities

As a Wellness Nurse at Sunrise Senior Living, your key responsibilities will include:

  • Conducting regular wellness visits to assess the health and well-being of our residents.
  • Developing and implementing individualized care plans to address the unique needs of each resident.
  • Collaborating with our healthcare team to ensure seamless communication and coordinated care.
  • Providing education and support to residents and their families on health-related topics and services.
  • Participating in quality improvement initiatives to enhance the quality of care and services we provide.

Requirements

To be successful in this role, you will need:

  • A current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Flexibility to work varied shifts, including weekends and holidays.
