Hamilton Health Care System is seeking a skilled LPN to join our team as a Care Navigator. As a key member of our healthcare team, you will be responsible for planning, supervising, and evaluating the nursing care of patients. You will work closely with physicians and other healthcare providers to deliver high-quality medical care to our patients. Your role will involve administering nursing care in an outpatient facility or clinic under the supervision of a registered nurse. You will participate in the delivery and evaluation of patient care, ensuring the health, comfort, and safety of our patients. Your experience and judgment will be relied upon to plan and accomplish goals, and you will perform a variety of tasks under general supervision. A certain degree of creativity and latitude is required.

**Key Responsibilities:**

* Observe professional ethics in maintaining confidential information acquired concerning patients and their families.
* Follow and enforce accepted safety practices for patients and the healthcare facility.
* Participate in performance improvement activities.
* Utilize the nursing process to oversee and provide age-specific patient care.
* Communicate effectively with patients and healthcare team members.
* Develop, implement, and update patient care plans.
* Educate patients and their families on healthcare information and rights.

**Qualifications:**

* Graduate of an approved school of nursing, as evidenced by verification of state licensure.
* A minimum of one year of experience as an LPN is preferred.
* Current TN LPN license and BLS certification are required.
