Responsibilities:

* Plan and implement activities that cater to the interests and needs of our residents, utilizing the HEART philosophy
* Conduct daily assessments to determine individualized programs and ensure resident engagement
* Facilitate socialization among residents and promote a sense of community
* Utilize the Staying Connected Form to promote activities and engage residents
* Provide input into each resident's assistance plan and contribute to their overall care
* Assist with decorations for special events, seasons, and holidays
* Plan and coordinate weekly outings and trips
* Identify and requisition necessary supplies for activities
* Provide daily assistance during meals and monitor resident behavior
* Assist with transportation for activity outings and group trips
* Perform other duties as assigned by supervisor to ensure smooth program operation

Requirements:

* Experience in activity planning with seniors preferred
* Thorough knowledge of dementia-related issues
* TRD certification, Associate's degree, or additional training in a related field preferred
* Minimum of 2 years' work-related experience in an environment focusing on dementia care, preferred
* Evidence of creativity, organizational skills, and a mature, pleasant, friendly, and cheerful personality
* Driver's license required with a clean driving record
