RN Hospice Care Coordinator

Lumina Hospice & Palliative Care, a reputable Oregon nonprofit organization, is seeking a skilled RN Hospice Care Coordinator to provide compassionate care and support to individuals and families facing serious illness or end of life.
Key Responsibilities:- Assess and evaluate patient needs, coordinating resources to meet those needs.
- Coordinate care provided by all hospice disciplines, ensuring comprehensive patient and family care.
- Provide nursing care as needed, addressing physical, emotional, and spiritual needs.
- Counsel patients and families on symptom control, medications, and direct nursing care measures.
- Assist patients and families in making informed decisions, providing alternatives and support.
- Attend to patient deaths, support families, and assist with final arrangements.
- Admit new patients, educate families, arrange services and equipment, and procure medications.
- Collaborate with healthcare providers, including physicians, pharmacies, and care facilities, to ensure optimal patient care.
- Communicate effectively with Lumina staff, functioning as an integral member of the patient care team.
- Develop and revise patient care plans in collaboration with IDT members, completing necessary documentation and attending IDT meetings.
- Commitment to serving the needs of the terminally ill.
- Current unencumbered Oregon licensure as a Registered Nurse.
- Nursing experience in hospice, palliative care, home health, or medical surgical care.
- Valid driver's license and acceptable driving record, with access to a reliable automobile.
- Excellent communication and documentation skills, with ability to work successfully with patients and families.
- Proficiency in electronic tools, including email, internet, and medical software.
- Flexibility, independence, and ability to work effectively as part of an interdisciplinary team.
- Ability to lift up to 40 pounds and perform physical demands of the role.
- Basic life support CPR certification or ability to obtain within two weeks of hire.
