About the Role:

Lumina Hospice & Palliative Care is seeking a compassionate RN Nurse Case Manager to join our team in Corvallis, Oregon. As a RN Nurse Case Manager, you will play a vital role in providing comprehensive patient and family care, ensuring the highest level of dignity and comfort for our patients.

Responsibilities:

  • Assess, evaluate, and regularly reevaluate the needs of patients and families, organizing resources to meet those needs.
  • Coordinate care provided by all hospice disciplines, ensuring seamless delivery of services.
  • Provide nursing care as needed, attentive to physical, emotional, and spiritual needs of patients.
  • Counsel patients and families regarding symptom control, medications, direct nursing care measures, nutrition, and other essential aspects of care.
  • Assist patients and families in making informed decisions by providing alternatives and support.
  • Attend deaths, support families, and assist with final arrangements.
  • Admit new patients, educate families, arrange services and equipment, obtain physician orders, and procure medications.
  • Collaborate professionally with other healthcare providers, including physicians, pharmacies, and care facilities, to ensure optimal care for patients.
  • Communicate effectively with other Lumina staff, functioning as an integral member of the patient care team.
  • Develop and revise patient care plans in collaboration with other IDT members, completing necessary documentation and attending IDT meetings for assigned patients.
  • Provide all services in full compliance with Medicare regulations, including timely and professional documentation.

Qualifications:

  • Commitment to serving the needs of the terminally ill.
  • Current unencumbered Oregon licensure as a Registered Nurse.
  • Nursing experience in hospice, palliative care, home health, medical surgical care, or equivalent.
  • Valid driver's license and acceptable driving record, with access to a reliable automobile for travel to and from patient homes.
  • Demonstrated skill in communication, patient documentation requirements, and service coordination, with the ability to work successfully with patients and family members in aspects of terminal care.
  • Solid skills in using electronic tools, such as email, internet, keyboarding, and medical software for accurate and timely charting.
  • Flexibility, independence, and demonstrated superior skills working with patients, families, and professional colleagues in delivering assistance within scope of practice.
  • Ability to work as a member of an interdisciplinary team.
  • Position requires ability to lift up to 40 pounds without assistance and to roll by pushing and pulling up to a 200-pound person who is able to assist or 150 pounds who is not able to assist.
  • Basic life support CPR card or ability to obtain a valid card prior to patient contact within two weeks of hire is required.
  • Bi-lingual, Spanish speaking ability is helpful.
