Senior Care Clinic Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our Senior Care Clinic team at the University of Washington. As a key member of our multidisciplinary team, you will provide high-quality patient care and work collaboratively with colleagues to deliver exceptional outcomes.
Key Responsibilities:- Provide in-person and phone triage, in-basket management, and develop clinical interventions and plans of care
- Transitions of care and interact with home health and community living providers
- Address patient concerns and provide education on safety issues relevant to seniors
- Active WA State or Multi-State RN License
- At least 2 years of professional acute care, med surg, or primary care nursing experience
