The organization's Street Outreach Program is seeking a dedicated individual to join its team as a Street Outreach Worker.


Key Responsibilities:
Conduct community outreach and engage with at-risk youth and their families
Assess immediate needs and develop individual service plans
Provide case management, service coordination, and aftercare services
Collect and document data, and represent the agency in relationships with other organizations

Requirements:
Bachelor's degree in Social Work or related field
Ability to work independently and exercise initiative
Experience working with youth and families, and sensitivity to diverse backgrounds and abilities
