Nurse Practitioner

Cantata Senior Living is seeking a dedicated and compassionate Nurse to join our team as a Staff Nurse PRN/Registry. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and providing exceptional service to our residents.
Key Responsibilities- Provide direct patient care, including administering medications, performing medical procedures, and developing and implementing individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Conduct thorough assessments, identify potential health risks, and develop strategies to mitigate them.
- Develop and maintain accurate and comprehensive patient records, ensuring compliance with regulatory requirements.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Provide education and support to patients, families, and caregivers, promoting health literacy and self-management skills.
- Stay up-to-date with the latest clinical research, best practices, and regulatory requirements, applying this knowledge to improve patient outcomes.
- Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
- Current CPR certification.
- Leadership ability and willingness to work harmoniously with and direct other personnel.
- Effective communication and interpersonal skills, with the ability to work with diverse populations.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Basic computer literacy and familiarity with electronic medical records (EMRs).
This role requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The successful candidate will be able to maintain a calm and professional demeanor, even in high-pressure situations.
