The Music Therapist plays a crucial role in delivering direct therapeutic services to patients in hospice and palliative care, as well as their families. The focus is on utilizing music as a therapeutic tool to enhance the quality of life for patients facing terminal illnesses, while also supporting their families through this challenging time. The Music Therapist collaborates with the interdisciplinary team to develop and implement comprehensive care plans tailored to the unique needs of each patient.

Key Responsibilities
  1. Conducts assessments of patients across all age groups to identify their needs and deliver appropriate interventions. This includes gathering data in partnership with patients and their families, as documented in patient records.
  2. Performs initial evaluations during the first visit, documenting findings within designated timeframes. Assesses emotional, physical, social, and cognitive aspects through musical interactions and communicates relevant findings to the care team.
  3. Designs and executes individualized care plans based on assessed needs, ensuring that interventions are developmentally appropriate and well-documented.
  4. Provides direct patient care in accordance with the established Care Plan, ensuring timely documentation of sessions. Develops music therapy sessions tailored to individual and group needs, offering both basic and specialized instruction as outlined in the Plan of Care.
  5. Regularly monitors and evaluates patient progress, making timely adjustments to the Care Plan as necessary. Recognizes and reports any changes in patient condition to the healthcare team.
  6. Identifies personal professional development needs and seeks guidance from supervisors to address these areas.
  7. Educates healthcare staff about the role and benefits of music therapy in hospice care.
  8. Participates in the development and evaluation of music therapy programs.
  9. Completes all mandatory training as required by the organization.
  10. Communicates effectively with clients, taking into account their age, cultural background, and individual needs.
  11. Demonstrates reliability in attendance and adherence to time-off policies, fulfilling annual education and medical requirements.
  12. Performs additional related duties as assigned.
Minimum Qualifications
  1. Bachelor's degree required; Master's degree preferred.
  2. Graduation from an accredited music therapy program; Board Certification as a Music Therapist.
  3. At least two years of experience in end-of-life care preferred.
  4. Valid Driver's License and auto insurance required.
  5. Access to reliable transportation.
