Our mission is to ensure that each resident receives the highest quality of service, promoting their functional independence and preparing them for a safe transition to the next level of care.
Key Responsibilities:
- Assign ICD-10 CM diagnosis codes accurately to residents' medical conditions.
- Manage case oversight for Medicare Part A and insurance processes.
- Conduct thorough assessments of nursing home residents' needs, tailoring personalized care plans based on these evaluations.
- Supervise the patient assessment workflow, ensuring timely and precise completion of assessments.
- Provide recommendations and submit assessments to the Department of Health, analyzing results to suggest policy improvements to management.
- Coordinate care plans in compliance with regulatory standards, ensuring that all necessary resources are accessible to residents.
- Develop and maintain schedules for Medicare and Medicaid, including initiating coverage for newly eligible patients and handling denial communications.
- Stay informed on regulatory updates related to Medicare coverage and assist in determining required documentation for Medicaid reimbursement.
Qualifications:
- Associate degree required; Bachelor's degree preferred.
- Registered Nurse with a valid license in good standing.
- In-depth knowledge of MDS processes.
- A minimum of one year of experience in MDS within a skilled nursing or long-term care environment.
- Exceptional attention to detail, with strong leadership and scheduling capabilities.
- Excellent communication skills, with a proven ability to collaborate effectively within a team.
- Proficient in MDS 3.0.
- Thorough understanding of applicable state and federal regulations.
- Certification as a Resident Assessment Coordinator.
