Nurse Care Coordinator

We are seeking a dedicated and compassionate Registered Nurse to join our team at Cantex Continuing Care Network. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to a positive and supportive work environment.
Job Summary:The Registered Nurse will be responsible for providing direct patient care, assessing patient conditions, and developing and implementing individualized care plans. You will work closely with our interdisciplinary team to ensure seamless care delivery and excellent patient outcomes.
Responsibilities:- Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
- Assess patient conditions, identify potential health risks, and develop and implement individualized care plans.
- Collaborate with the healthcare team to ensure comprehensive care delivery and excellent patient outcomes.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
- Participate in quality improvement initiatives and contribute to a culture of safety and excellence.
- Graduation from an accredited nursing program.
- Valid Registered Nurse (RN) license.
