Physician - CenterWell Primary Care Port Arthur

About the Role:
CenterWell Primary Care is seeking a skilled Physician to join our team in Port Arthur. As a Physician, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities:
- Provide primary care services to patients, including wellness visits, physical exams, and chronic condition management.
- Develop and implement treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Communicate effectively with patients, families, and other healthcare providers to ensure seamless care coordination.
Requirements:
- Current and unrestricted medical license.
- Graduate of an accredited MD or DO program.
- Excellent verbal and written communication skills.
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
- Fully engaged in the concept of "Integrated team-based care" model.
- Willingness and ability to learn/adapt to practice in a value-based care setting.
- Superior patient/customer service.
- Basic computer skills, including email and EMR.
Preferred Qualifications:
- Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine.
- Active and unrestricted DEA license.
- Medicare Provider Number.
- Medicaid Provider Number.
- Minimum of two to five years directly applicable experience preferred.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.
