Job Summary:
We are seeking a skilled Senior Clinical Social Worker - Mental Health Specialist to join our team. In this role, you will be responsible for providing comprehensive clinical social work services to patients and families, including assessment, treatment, and discharge planning. You will work closely with multidisciplinary teams to ensure seamless care coordination and effective communication.
Responsibilities:
• Provide psychosocial assessments and develop treatment plans in collaboration with patients, families, and healthcare teams.
• Conduct crisis interventions and provide support to patients and families in crisis situations.
• Develop and implement in-home care plans, exploring alternatives to in-home care and arranging for placement as needed.
• Provide individual, family, or group therapy, education, and counseling to promote optimal health outcomes.
• Collaborate with healthcare teams to promote a smooth, coordinated plan of care and address psychosocial factors affecting patient recovery.
• Identify and access community resources to support patients and families.
• Develop and implement quality improvement activities and research to enhance service delivery and effectiveness.
Requirements:
• Master's degree in Social Work from an accredited institution.
• Licensed Independent Clinical Social Worker (LICSW) or equivalent licensure.
• Five (5) years of related experience in mental health settings.
Preferred Qualifications:
• Experience working with major mental illness, dementia, and the elderly.
Physical Requirements:
• Normal office setting.
• Inpatient and/or outpatient clinic patient care setting.
• Frequent sitting, occasional standing & walking, and lifting of 5-10 lbs.
Skills & Abilities:
• Strong diagnostic, treatment, interpersonal, organizational, and communication skills.
• Ability to read/write and communicate in English.
• Excellent interpersonal skills.
• Ability to function well in very busy situations.
• Responsible and reliable.
• Good organization skills.
Estimated Salary: $80,000 - $110,000 per year
