Role Summary: The Community Living Navigator plays a critical role in helping high-need individuals navigate the mental health system. As a member of our Outreach and Navigation Team, you will work collaboratively with community partners to ensure seamless coordination of care and promote healthy coping mechanisms.

**Key Responsibilities:** Provide essential information and resources to high-need individuals, helping them understand their options and access necessary services. Collaborate with community partners, including healthcare providers, social services agencies, and law enforcement, to ensure coordinated care. Develop and implement personalized treatment plans that address the unique needs of each individual. Conduct regular outreach activities to engage with individuals in various settings, promoting healthy coping mechanisms and self-identified quality of life. Establish strong relationships with adult serving systems, responding in a culturally responsive, trauma-informed, and strength-based manner.

Requirements: BA/BS in Human Services or related field with 2 years of relevant professional experience or equivalent education and work experience in the mental health, substance use, developmental disability, and/or criminal justice systems. Satisfactory completion of all required background screenings. Full vaccination against COVID-19 virus. Reasonable accommodations will be made for qualified religious or medical/disability needs. Excellent written and verbal communication and interpersonal skills. Experience working with individuals with disabilities. Individuals with lived experience in the mental health, substance use and/or criminal justice systems are encouraged to apply.

About CCSI: We have a market competitive salary of $60,000 - $80,000 per year, depending on experience.
