Job Summary

We seek an experienced Registered Nurse to join our team as a Nurse Navigator, supporting direct patient care in a non-direct physical role. This innovative position integrates the nurse with onsite telemedicine into the acute inpatient care model, allowing for superior patient monitoring and expert nursing consultation. The successful candidate will utilize their clinical knowledge and critical thinking skills to coordinate and implement care plans, ensuring seamless transitions across the continuum.

Key Responsibilities
  • Collaborate with bedside staff, patients, families, and medical teams to ensure optimal provision of care through digital platforms.
  • Assess, plan, implement, coordinate, monitor, and evaluate patient care plans from admission to discharge.
  • Utilize clinical knowledge and critical thinking skills to coordinate and implement care plans.
  • Ensure seamless transitions across the continuum.
  • Monitor quality metrics specific to the department and maintain accurate documentation in patient records.
Requirements
  • Graduation from an accredited school of nursing and current licensure in the State of New York.
  • Minimum of 8 years' experience in adult health condition management.
  • Electronic Medical Record (EMR) experience.
  • Current or recent Bedside RN experience.
Salary & Benefits

We offer a competitive salary range of $40.10-$55.85 per hour, based on experience, skills, and education.
