About the Position

We are seeking a highly skilled Documentation Excellence Manager to join our team at Trilogy Health Services, LLC. As a Documentation Excellence Manager, you will be responsible for managing and maintaining clinical quality documentation for healthcare organizations.

Key responsibilities include ensuring compliance with regulatory requirements and standards by developing and implementing documentation processes and procedures. You will also review and analyze documentation for accuracy, completeness, and adherence to guidelines.

The estimated salary for this position is $85,000 - $130,000 per year, depending on experience and location.

Main Responsibilities

  1. Develop and implement clinical excellence initiatives
  2. Lead the development of documentation processes and procedures to ensure compliance with regulatory requirements and standards
  3. Review and analyze documentation for accuracy, completeness, and adherence to guidelines
  4. Collaborate with cross-functional teams to identify areas for improvement and implement corrective actions
  5. Provide clinical content expertise and knowledge of care provider systems to create solutions that will impact quality across the care continuum
  6. Create and assist in the development of accurate documentation in MatrixCare
  7. Troubleshoot and assist in creating an action plan for quality improvement
  8. Establish the strategic direction to incorporate division based multi-disciplinary teams that are focused on reducing variations and improving clinical performance
  9. Support state surveys and readiness efforts
  10. Analyze trends, quality data and identify opportunities for performance improvement initiatives in quality and safety indicators
  11. Develop and provide education related to quality measures, processes, and procedures
  12. Conduct analysis to quantify the impact of policy and/or market changes
  13. Manage automated reporting to provide timely information for informed decision-making
  14. Review medical records for new admissions to ensure proper documentation
  15. Use critical thinking skills to identify missing documentation and communicate effectively with stakeholders
  16. Track, analyze and advise on infection, fall and wound events to improve patient outcomes
  17. Develop tools to optimize clinical quality and reporting
  18. Provide leadership and direction for clinical data systems
  19. Maintain current systems and eliminate outdated reporting
  20. Collaborate with various departments to achieve goals and objectives
  21. Complete external audit requests and upload them in a timely manner
  22. Perform other duties as assigned

Requirements

  • High School / GED
  • 1-3 years of experience
  • No licenses or certifications required
