Job Summary

Leaders for Life is seeking a highly skilled and compassionate Mental Health Specialist to join our team. As a key member of our Mental Health Skill-building (MHSS), Intensive In Home (IIH) program and/or Crisis Programs, you will work closely with adults with serious mental illnesses and co-occurring substance use, as well as children with behavioral problems.

Key Responsibilities
  • Caseload Management: Maintain a caseload of 4 clients from either program, providing individualized support and services.
  • Service Planning: Develop and implement individualized service plans with ongoing documentation of progress, ensuring clients receive the necessary resources and support.
  • Community Linkages: Link individuals to community resources as needed, fostering a network of support and connection.
  • Case Coordination: Collaborate with agencies to ensure seamless care coordination and effective communication.
  • Daily Living Skills: Train individuals on daily living skills, promoting independence and self-sufficiency.
  • Supervision and Training: Participate in regular supervision with clinicians and attend staff meetings and trainings to stay up-to-date on best practices.
  • Progress Notes: Complete and submit daily progress notes, providing a clear record of client progress and services provided.
  • Crisis Intervention: Provide on-call crisis intervention services to IIH clients as necessary, ensuring timely and effective support.
  • Collaboration: Establish and maintain effective working relationships with other community agency professionals, promoting a culture of collaboration and cooperation.
Requirements
  • Education: At least a bachelor's degree in a human services field.
  • Certification: Registered as QMHP-A, C, or T through the VA Board of Counseling.
  • Experience: A minimum of one year working with adults/children with severe diagnoses.
