About the Role

We are seeking a highly skilled and compassionate Speech Language Pathologist to join our team at CenterWell Primary Care. As a key member of our home health care team, you will play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.

Key Responsibilities
  • Evaluate and provide speech and language pathology services to patients in their homes or facilities
  • Develop and implement individualized treatment plans to address patients' unique needs and goals
  • Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement
  • Provide education and training to patients and their families on effective communication strategies and techniques
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Master's degree in Speech Language Pathology or related field
  • Current certification in Speech Language Pathology (CCC-SLP) or equivalent
  • Minimum of 6 months of experience as a Speech Language Pathologist
  • Home health experience preferred
  • Current and unrestricted license to practice as a Speech Language Pathologist
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
